Part 1: Optimizing ribavirin therapy in transplant recipients with a chronic hepatitis E virus infection by devoloping a NONMEM model for ribavirin. Part 2: Improving immunosuppressive therapy in liver transplant recipients by performing 2 RCT's in which different immunosuppresive strategies will be compared to modify the tacrolimus related toxicity in this population. Part 3: Evaluate the contribution of an outpatient medication service for liver transplant recipients.
RHYME-RCT is a multi-center randomized controlled trial to identify and improve non-adherence in patients with resistant hypertension and are included from 10 peripheral and 2 academic hospitals. Non-adherence is measured by means of a validated dried blood spot method. This method includes 17 antihypertensive drugs from different drug classes.
The aim of the thesis is to identify and examine strategies regarding the preparation and administration of medication to improve medication safety in hospitals. Examined interventions:
Outpatient Parenteral Antimicrobial Therapy (OPAT) service provides antimicrobial therapy by parenteral infusion without hospitalization. Vancomycin is a widely used antibiotic in OPAT, however a drawback of its application in OPAT is the need for traditional blood sampling at a blood sampling facility for therapeutic drug monitoring. By developing a dried blood spot sampling method (i.e. a finger prick), enabling patients to collect their own sample at home, the costs of OPAT may be further reduced and patient satisfaction may be increased.
