Ontwikkeling van een toolbox, waarmee 1) in de apotheek patiënten met een hoog valrisico geselecteerd kunnen worden; 2) interventies toegepast kunnen worden door de apotheker om valrisico bij individuele of groepen patiënten te verlagen, onder andere door middel van het afbouwen van valgevaarlijke geneesmiddelen.

The aim of the thesis is to identify and examine strategies regarding the preparation and administration of medication to improve medication safety in hospitals. Examined interventions:

1. central automated dose dispensing combined with barcode-assisted medication administration,
2. a pharmacy-based centralised intravenous admixture service.

1. Cost benefit analysis clinical medication reviews
2. Follow up of intervetnions from clinical pharmacists.
3. Medication information needs of hospitalized patients.
