Dit project kijkt naar het effect van doorgebruikt thuismedicatie en medicatie in eigen beheer op medicatieveiligheid, kosten en patiënttevredenheid. Ook wordt er gekeken naar randvoorwaarden voor goede implementatie van deze interventies.

The aim of the thesis is to identify and examine strategies regarding the preparation and administration of medication to improve medication safety in hospitals. Examined interventions:

1. central automated dose dispensing combined with barcode-assisted medication administration,
2. a pharmacy-based centralised intravenous admixture service.

1. Cost benefit analysis clinical medication reviews
2. Follow up of intervetnions from clinical pharmacists.
3. Medication information needs of hospitalized patients.
