The (positive) effects of antihypertensive drugs in the cardiovascular risk management are undoubted. However, like all drugs, antihypertensive drugs also have negative side-effects. Especially the elderly, are more susceptible for these adverse effects. It's not clear whether the benefits outweight the risks. So is it possible to deprescribe antihypertensive drugs and minimise risks for adverse events, while maintining blood-pressure levels in accordance with guidelines?

The aim of the thesis is to identify and examine strategies regarding the preparation and administration of medication to improve medication safety in hospitals. Examined interventions:

1. central automated dose dispensing combined with barcode-assisted medication administration,
2. a pharmacy-based centralised intravenous admixture service.

1. Cost benefit analysis clinical medication reviews
2. Follow up of intervetnions from clinical pharmacists.
3. Medication information needs of hospitalized patients.
